I have solved the problem. After diassembling the cutoff valve it was clear that the valve had been incorrectly manufactured. The valve has a long steel shuttle that has a pin at the end that opens or closes a small hole (about 1mm dia). The problem was that the one of the two hooks on the brass piece retaining it to the steel shuttle was shorter than the other one. This resulted in the shuttle being unable to move away from the hole and open the valve. Removal of about 0.5mm of metal from the shoulder of the shuttle resulted in more shuttle travel and the valve could open. This modification fixed the problem with the valve failing to open and confirmed that the problem was due to a manufacturing problem.
